Requirements
- Birth Certificate (both original and clear photocopy)
- Baptismal Certificate (clear photocopy)
- Parents’ Marriage Contract (clear photocopy)
- ACR / ICR for non-Filipino citizen
- Pictures (size: 1×1) for the IDs
- Student – 4 copies
- Parents – 3 copies each
- Possible Fetchers (4) – Caregivers/relatives (2 copies each)
- Photocopy of 2 Valid IDs (Parents)
